Protocol for Physician Referrals

1. Prepare pertinent information needed i.e. prior to initiating the Direct Admission Process, patient demographics, clinical information to support the level of care requested, authorizations where applicable, etc.
2. Submit admissions request via telephone, fax, or electronically (electronic forms coming soon)

3. Please provide a direct call back number for possible clarifications from the Patient Access Department and for a potential call from the Utilization
